As a nurse you have to get personal details from your patients and fill in a hospital registration form.
In patient record:
  • first name:isim

  • gender/male-female:cinsiyet/erkek-kadın
  • D.O.B:date of birth
  • occupation:işi
  • marital status:medeni hali
  • next of kin:en yakını
  • contact no:iletişim no
  • smoking/alcohol intake:sigara/alkol kullanımı
  • reason for admission:başvuru sebebi
  • allergies
  • GP:aile hekimi
  • medical history:past illnesses and injuries

And before you collect data, you can say:
''Its time for me to assess your health condition''
How can i address you?(But when you say adressing you mean how to call you such as miss,mrs,sir)
And you can ask questions just like i am gonna give as an example below:
  • Have you ever had measles ?
  • Are you allergis to certain food or medication?
  • Have you ever been immunized against polio?
  • Do any of your close family suffer from any of the following Diabetes,HIV,TBC,mental illness?
  • How many do you smoke a day?
  • Do you have any health insurance?
  •  And you can create a lot of sentences like these.

For example you have a patient and she/he has pain.What do you ask to her/him?

  • When did your pain start?
  • How often does it occur?
  • How its intensity changed?(Yoğunluğu,miktarı nasıl değişti)
  • How long does it last?
  • Does it move around?
  • What was the worst it has been?
  • Does this interfere your usual activities?In what way?
  • Have you taken anything for it?
  • Does it appear in particular time?
